Model Release Form

The following is an agreement between ________________________, hereafter referred to as the “client” and JP Portrait Studio, hereafter referred to as the “photographers”.  The term “studio” is used to describe the location (or locations) at which client has requested the photographers perform professional services.  Client attests to their authority to grant permission to use the premises and agrees to indemnify and hold harmless the photographers from any damages to real or intellectual property arising from the use or misuse of said premises.

In consideration of payment for services rendered, the client does herby grant the photographers an exclusive, non-revocable and fully paid license to duplicate client’s image or likeness for any legal purpose, including (but not limited to) the display of finished photographs in the online and print portfolios of the photographers, advertisements and other marketing materials.
